  • What is the NDSS?

    The National Diabetes Services Scheme (NDSS) is a government initiative providing support and services to people living with diabetes so they can better understand and self-manage their condition. 

    The NDSS provides free and subsidised products and services to people living with diabetes who are registered with it. Free products include syringes and pen needles, while subsidised products include blood and urine testing strips. Find out more about the NDSS. 

  • Who can register for the NDSS? 

    Registration with the NDSS is free and open to all Australians who are: 

    • Residents of Australia 
    • Have been diagnosed with diabetes by a medical practitioner, and 
    • Who hold a current Australian Medicare card or Department of Veterans’ Affairs file number. 
  • How do I register for the NDSS? 

    Registration with the NDSS is free and open to all Australians who are: 

    • Residents of Australia 
    • Have been diagnosed with diabetes by a medical practitioner, and 
    • Who hold a current Australian Medicare card or Department of Veterans’ Affairs file number. 

    To register, you need to: 

    • Complete the registration form. If you are under 15 years old or are an adult receiving continuing care, your primary guardian or carer will need to complete the ‘Guardian or carer’ section of the form 
    • Have the registration form certified by a registered medical practitioner such as your doctor, endocrinologist or credentialled diabetes educator, and 
    • Email the registration form to ndss@diabetesaustralia.com.au or send it by mail to: 
    • Diabetes Australia, GPO Box 9824 in your capital city.

  • What is the difference between my NDSS card and Diabetes Queensland Community membership?

    Your National Diabetes Services Scheme (NDSS) card places you on a national register of people living with diabetes.

    Being a member of Diabetes Queensland’s Live Your Life Community connects you to others living with diabetes and gives you access to information, events, resources and support to help you manage your diabetes.

  • What are the different types of membership Diabetes Queensland offers? 

    Live Your Life Community membership is for people living with all types of diabetes (including special concession discounts).

    Full Membership costs $60 

    Concession Membership  which includes DVA Gold card, Pensioner Concession Card, Health Care Cards, Commonwealth Seniors Health Care Card, Student, costs $33

  • Is there an annual membership fee to join the Diabetes Queensland community?

    Yes there is an annual fee, which covers your community membership from July 1 to June 30 the following year.

    Full Membership costs $60 annually

    Concession Membership which includes DVA Gold card, Pensioner Concession Card, Health Care Cards, Commonwealth Seniors Health Care Card, Student costs $33 annually. 

     

  • Am I eligible for a concession discount when I join the Live Your Life Community? 

    The following cards entitle you to a concession discount: 

    • DVA Gold card  
    • Pensioner Concession Card  
    • Health Care Cards  
    • Commonwealth Seniors Health Care Card  
  • How can I pay for my Community membership? 

    Online: payment for new and renewing members can be made online using your credit card. 

    Post: you may also post a completed application or renewal form to Diabetes Queensland at GPO Box 9824, Brisbane QLD 4001. Payment can be made via cheque (payable to Diabetes Queensland) or credit card.  Please ensure you do not send cash through the post. 

    Fax: fax through a completed application or renewal form with your credit card details to (07) 3506 0909.  

    Phone: phone our Helpline on 1800 177 055 to have your payment processed by one of our friendly staff. Be sure to have your credit card details ready. 

    BPay: renewing members have the option of paying via BPay using the details provided on your renewal form 

    Australia Post – PostBillPay: renewing members have the option of paying in person via Australia Post BillPay at any Australia Post outlet.  

  • When do I renew my Community membership?

    Live Your Life Community membership renewals are issued in May each year. We will send you a notice to renew in the mail and if we have your email address you will receive an electronic renewal notice.

    Renewal payments are due on 1 July.

    The membership year runs from 1 July to 30 June the following year.
